To request an appointment please call 305-273-9377 Mon-Friday 9am-5pm

Be sure to have your insurance information with you when you call. If your insurance plan requires a referral from your primary care provider, it is the patient’s responsibility to request this and ensure he/she brings it to their appointments. We will not be able to see you without a referral, if your insurance requires one.

New Patients

Call us right away!

Please plan to arrive 30 minutes before your scheduled appointment time to complete paperwork.  New patient information packets are available here for you to complete prior to your appointment. We recommend that you take the time to print and complete these to expedite the check-in process.
To your first visit please be sure to bring:

Insurance Card & Referral (if required by your insurance company)
Drivers License or Photo Identification
Current Laboratory Blood & Urine Results from your primary care physician
Any Ultrasound or Imaging test results performed by your primary care physician
All Current Medications you are taking. Please bring the medications rather than just a list to assure accuracy.

Confirming and Canceling An Appointment

Our automated system will call to remind you of your appointment. Please ensure you have provided us with a valid home phone or cell number, and please do not hang up, listen to the recording in its entirety as important information will be provided. If you must cancel a scheduled appointment, please notify us at least 48 hours in advance.

Telephone Calls

We try to return calls, as soon as possible. Calls received after 4pm will be returned the following business day. Please note, doctor calls will say “blocked” “private” or “no caller id”, please make sure you pick up to avoid important missed calls from your physician.

Lab Testing and Test Results

Our physicians will request that you repeat your blood work before your next scheduled follow-up appointment. Please ensure that you complete these at the laboratory draw station that is covered by your insurance company and make sure you complete these as instructed. This will ensure that we have the results available in our office for the physician to discuss with you, at the time of your next appointment.

Copies of your labs will be available upon request at least a week after they are drawn.If there are any serious problems with your laboratory tests,we will attempt to contact you as soon as possible.

Prescription Refills

Refills will take 48-72 hours to process, please do not wait until you are completely out of your medication to call our office, allow ample time for your request.

Your pharmacy will need to fax in the refill request to 3052739388, we do not take these over the phone.
